Jose Mourinho is reportedly planning an audacious raid on his former club Chelsea to bring Eden Hazard to Old Trafford.

The Belgian star has won two Premier League titles at Stamford Bridge since joining in 2013, but could be in line for a move this summer, with Real Madrid thought to be the favourites to land his signature.

Hazard reportedly had a strained relationship with Mourinho during their time together at Chelsea, which ended when Mourinho was sacked before Christmas in 2015, but The Sun is reporting that the attacking midfielder is now the Man United manager's top transfer target.

The paper says that Hazard has turned down a bumper contract extension and Mourinho is ready to pounce with a British record offer to land the 26-year-old.

Hazard joined Chelsea in the summer of 2012, with Mourinho returning to the club the following season.

The duo won the Premier League and a League Cup together before the Portuguese departed.

Hazard wouldn't be the first of Mourinho's former Chelsea stars to join him at Old Trafford, with Nemanja Matic making the switch last summer.
