Ian O'Doherty: There but for the grace of God . . .

As you know, we now live in a 24-hour rolling news cycle, where the race to be the first to break news is more competitive than it has ever been before.

But in this frenetic environment, with incredible pressure to fill space and hit deadlines, there are times when things will, inevitably, go ever so slightly pear-shaped.
