STOLEN childhoods, missed opportunities and official regret. The tragic themes of the latest inquiry into children failed by the State have all been aired before, too many times now.

In 1993 there was the Kilkenny incest case. Three years later came the bleak life of the late Kelly Fitzgerald in Mayo.

In 1998 came the plight of six McColgan children in Sligo, beaten and abused by their father.
