HSE to probe patient river death
HEALTH bosses are to carry out a review after an elderly hospital patient died after falling into a river.
Sean Hyland (88), who was a patient at the Clogheen District Hospital in South Tipperary, was discovered missing from the ward on Monday.
He died after falling into the River Tar close to Clogheen at around 2pm.
A Health Services Executive spokeswoman said: "All appropriate measures were implemented immediately in that searches were undertaken by staff and gardai and family were notified."
She said a review would now be undertaken by the HSE as was standard practice.
Gardai said it was being treated as a tragic incident.
