Health Minister Dr James Reilly has pledged to meet the husband of Savita Halappanavar following the publication of the damning inspection report into her death.
He said he had not been able to meet him since the inquest into Ms Halappanavar's death last April.
"I have met him twice," Dr Reilly said. "For one reason or another, our paths didn't cross or it didn't suit, but I am certainly available and I will be more than happy to meet with him," he said.
Dr Reilly said that the matter of medical inquiries into the staff in University College Hospital Galway was for the Medical Council.
He said that lessons had to be learned from the catastrophic failures identified in the HIQA report into Halappanavar's death.
"As I say, I deeply regret that our health service has failed his (Praveen Halappanavar's) wife in such a terrible way," he said.
